Base Salary: $315,900 $386,100

Are you passionate about providing holistic, team-based care for seniors? As a Primary Care Physician, youll play a central role in shaping the health outcomes of older adults in a collaborative and mission-driven environment. This is more than a clinical roleits a chance to impact lives every day through relationship-based care and thoughtful coordination.

Your Role at a Glance
As a core member of our Interdisciplinary Team (IDT), youll lead the delivery of person-centered care by:

  • Performing comprehensive, in-person health assessments for all participants

  • Developing individualized care plans in partnership with the IDT

  • Managing care across settingshome, nursing home, and hospitalto ensure seamless transitions and consistent support

  • Overseeing admits and discharges for short-term, acute, and long-term care placements

  • Guiding and mentoring Nurse Practitioners and other care team members

  • Participating in rotating call coverage (nights/weekends) to support 24/7 continuity of care

What Youll Bring

  • MD or DO with successful completion of an ACGME-accredited residency

  • Board Certification in Internal Medicine or Family Practice (Geriatrics preferred)

  • Active, unrestricted California Physician License

  • DEA Registration and State CDS Certification

  • Valid drivers license and a clean driving record

  • Proof of current auto insurance

  • A commitment to whole-person, dignified care for the senior population
