Position Summary:

Ellenhorn is seeking a dedicated and compassionate PACT Clinician to provide psychosocial support as part of a multidisciplinary team to individuals with psychiatric experiences and mental illness. The ideal candidate will serve as a primary case manager for a small caseload and provide 1:1 and group sessions in areas such as wellness, vocational support, family support, spirituality, social interactions, and addictions, primarily in a community-based setting.

Salary: $70,000 $90,000

Key Responsibilities:

  • Provide service coordination (case management) for an assigned group of clients, including coordinating and monitoring the activities of the individual treatment team (ITT).
  • Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the client and the ITT.
  • Provide individual side-by-side support, counseling, therapy, and symptom management, ensuring immediate changes are made in the treatment plan as clients needs change.
  • Educate and support clients families, advocating for clients rights and preferences.
  • Conduct holistic and comprehensive assessments with other team members.
  • Act as liaison and consult with community agencies and families to maintain coordination in the treatment process.
  • Provide education and opportunities for other team members to engage in and learn about expressive therapies work (in-service training, staff wellness days, etc.).
  • Participate in on-call rotation for crisis intervention/support, available 24/7.
  • Document client progress to maintain a permanent record of client activity according to established methods and procedures.

Non-Essential Functions:

  • Occasional interstate travel.
  • Perform other related duties as assigned.

Qualifications Required:

  • Masters degree in a relevant field.
  • Post-secondary education and training, including internships and supervised practical experiences in clinical or rehabilitation settings with individuals with severe and persistent mental illness.
  • Must be a senior clinician, licensed or certified to work in the field (LICSW, LMHC, or LMFT required).
  • Commitment to ethical standards and continuous professional development.
  • Ability to establish supportive, trusting relationships with clients, their families, and their networks.
  • Must have a valid drivers license, dependable automobile, and a clean drivers record.

Qualifications Desired:

  • Experience working with individuals with addictions and mental illnesses.
  • Proficiency in client-centered practices such as Open Dialogue, Narrative Therapy, Internal Family Systems, Motivational Interviewing, Harm Reduction, Emotionally Focused Therapy, and Acceptance & Commitment Therapy.

Physical Demands and Work Environment:

  • Physical Demands: Occasionally required to walk, sit, type, handle objects, reach, bend, walk up and down stairs, talk, hear, and see. Ability to occasionally lift/move up to 50 pounds.
  • Work Environment: Most work is performed outside of the office setting in the community.
