Key Responsibilities

Conduct a Speech and Language Pathology evaluation or screen. · Selects and administers the appropriate diagnostic procedures to determine the extent of the patients impairment and the prognosis for improvement. · Develop an individualized Plan of Care with functional goals. · Provide appropriate therapeutic interventions. · Evaluate patient response to treatment and modify plan as necessary · Document the course of patient care including progress made and the continuing need for treatment. · Provide patient and/or family education to help achieve goals. · Adheres to KHI Attendance Policy. · Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others. · Adhere to policies, rules, and regulations from KHI, Arizona State Board of Speech and Language Pathology, and Medicare. · Assists with orientation of new staff as requested. · Meets KHI requirements for skills competency and safety. · Participate in continuing education/professional development activities. · Assists with the SLP students and Clinical Fellow program. · Participate in staff meetings, client relation and marketing activities. · Documentation is complete and accurate, meeting all KHI, Arizona State Board of Speech Language Pathology, and Medicare regulatory requirements. · Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation. · Individual productivity meets department standards. · Patient satisfaction will meet or exceed department standards.

Qualifications

  • Education: Graduate of a Masters Degree program, or foreign equivalent, in Speech & Language Pathology.
  • Licensure: Current Arizona License for Speech & Language Pathology
  • Certification: BLS and AED through the American Heart Association
  • Certification: Certificate of Clinical Competency (or eligible) Experience: Recent experience working with an adult and geriatric population

Preferences Education/Experience: Serving people with physical disabilities is desirable. nce working with an adult and geriatric population

Work Requirements

The Incumbent shall have the ability to lift and transfer patients and equipment, have the ability to stand, walk, sit, bend, stoop, crouch, and communicate to provide therapeutic interventions.
