Job Title: Licensed Practical Nurse (LPN) - Developmental Disabilities Residential Care
Location: Multiple residences within the Philadelphia area
Shift: 3 PM - 11 PM
Salary: $35 per hour
Type: Temp to Perm, W2 position with full benefits

Summary:
The ideal LPN candidate will oversee, coordinate, and directly address health-related concerns of program participants. This role requires a strong sense of responsibility, dedication to patient care, and the ability to travel to different locations throughout their shift.
Essential Duties and Functions:
- Maintain accurate and current nursing documentation and medication administration records.
- Educate and assist residents with medication management and adherence to physician orders.
- Provide various nursing care services, including but not limited to wound care, trach care, and diabetic counseling.
- Assess the health and emotional needs of residents and adjust care plans accordingly.
- Communicate efficiently with the nursing team, program staff, and management regarding resident concerns.
- Uphold the highest level of confidentiality in all aspects of the job.
Credentials Required:
- Valid Licensed Practical Nurse (LPN) license.
- Pennsylvania Criminal Clearance (obtained within the past year).
- Current CPR certification.
- Proof of Covid-19 Vaccination.
- Valid driver's license.
- Recent Physical and TB or Quantiferon test (within the last 6 months).

Qualifications:
- Demonstrated experience as an LPN, preferably in a residential or community health setting.
- Exceptional assessment, organization, and problem-solving skills.
- Ability to operate independently and as part of a multidisciplinary team.
- Effective communication skills with a strong emphasis on confidentiality.
- Must possess reliable transportation for travel between residential locations.
