Job Title: Licensed Practical Nurse (LPN) - Developmental Disabilities Residential Care

Location: Multiple residences within the Philadelphia area

Shift: 3 PM - 11 PM

Salary: $35 per hour

Type: Temp to Perm, W2 position with full benefits

Summary:

The ideal LPN candidate will oversee, coordinate, and directly address health-related concerns of program participants. This role requires a strong sense of responsibility, dedication to patient care, and the ability to travel to different locations throughout their shift.

Essential Duties and Functions:

  • Maintain accurate and current nursing documentation and medication administration records.
  • Educate and assist residents with medication management and adherence to physician orders.
  • Provide various nursing care services, including but not limited to wound care, trach care, and diabetic counseling.
  • Assess the health and emotional needs of residents and adjust care plans accordingly.
  • Communicate efficiently with the nursing team, program staff, and management regarding resident concerns.
  • Uphold the highest level of confidentiality in all aspects of the job.

Credentials Required:

  • Valid Licensed Practical Nurse (LPN) license.
  • Pennsylvania Criminal Clearance (obtained within the past year).
  • Current CPR certification.
  • Proof of Covid-19 Vaccination.
  • Valid driver's license.
  • Recent Physical and TB or Quantiferon test (within the last 6 months).

Qualifications:

  • Demonstrated experience as an LPN, preferably in a residential or community health setting.
  • Exceptional assessment, organization, and problem-solving skills.
  • Ability to operate independently and as part of a multidisciplinary team.
  • Effective communication skills with a strong emphasis on confidentiality.
  • Must possess reliable transportation for travel between residential locations.
