Therapist
Job Description:
The Therapist is responsible for counseling individuals regarding psychological or emotional problems related to substance abuse and/or mental health.
1. Counsel individuals regarding psychological or emotional problems related to substance abuse and/or mental health.
2. Provide group, and family counseling sessions as required.
3. Develop and implement therapeutic plans in compliance with standards for state and federal agencies and all other applicable rules and regulations.
4. Serve as Primary Therapist for a revolving caseload.
5. Work closely with clinical team to ensure continuity and quality of care for clients.
6. Maintain clients records within the EMR including DAP notes, clinical treatment plans, treatment plan reviews, transfer summaries, ASAMs, discharge summaries, etc.
7. Coordinate with client, family and referral sources for discharge planning.
8. Communicate with clinical support staff for evaluations and ancillary services for psychiatric, medical and biofeedback.
9. Maintains confidentiality pertaining to clients and families.
10. Any other duties as instructed by management.
11. Acquire and maintain all CEU, CPR, trainings, etc. as required by DMHA and JCAHO.
Masters Degree in Psychology, Counseling, Social Work or other related degree
Active LMHC or LCSW
