The Director of Case Management is responsible for the oversight of Case Management unit, including but not limited to clinical resource management, discharge planning activities, patient advocacy, clinical social work and best practice in medical necessity determination and concurrent review. Accountable for the execution, adaptation and outcome of care management as prescribed in the initial Utilization Review Plan with regard to resources, appropriateness of care and adherence to a designated Geometric Length of Stay (GLOS) for each patient.
Must Haves:
Minimum Education:
- Associate's Degree in Nursing (required).
- Bachelors Degree in Nursing or Masters Degree in a related field (preferred).
- Current RN license to practice in North Carolina (NC licensure or multi-state compact license).
Required Skills & Certifications:
- BLS Certification (required).
- ACMA Certification (preferred).
- CCM Certification (must be completed within the first year of employment or transfer into the role).
Key Responsibilities:
- Oversee the Case Management unit, including clinical resource management, discharge planning, patient advocacy, clinical social work, and best practices for medical necessity determination and concurrent review.
- Ensure care management aligns with the Utilization Review Plan, focusing on resource allocation, appropriateness of care, and adherence to designated Geometric Length of Stay (GLOS) targets for patients.
This role requires a skilled leader with expertise in care coordination, resource optimization, and patient-centered outcomes.
