Job Overview:

As the Care Management Services Director, you will be responsible for overseeing the day-to-day operations of our care management services. This includes managing a team of care managers, ensuring the delivery of high-quality care to our clients, and maintaining strong relationships with our healthcare facility partners. You will also be responsible for developing and implementing strategies to improve the overall efficiency and effectiveness of our care management services.

Key Responsibilities:

Manage and provide leadership to a team of care managers, including hiring, training, and performance evaluations

Oversee the delivery of care management services to clients, ensuring that all care plans are implemented effectively and efficiently

Develop and maintain strong relationships with healthcare facility partners, including regular communication and collaboration to ensure the best possible care for clients

Monitor and analyze data related to care management services, identifying areas for improvement and implementing strategies to increase efficiency and effectiveness

Ensure compliance with all regulatory requirements and company policies and procedures

Collaborate with other departments within the agency to improve overall operations and customer satisfaction

Stay up-to-date with industry trends and best practices in care management services, and make recommendations for improvement as needed

Qualifications:

Bachelor's degree in healthcare administration, business, or a related field, and a Current state RN License.

Minimum of 5 years of experience in a leadership role in the healthcare industry, preferably in care management services

Strong knowledge of care management principles and best practices

3-5 years in a supervisory role.

Excellent communication and interpersonal skills, with the ability to build and maintain relationships with clients and healthcare facility partners

Proven leadership skills, with the ability to motivate and manage a team effectively

Strong analytical and problem-solving skills

Knowledge of relevant regulations and compliance requirements

Proficient in Microsoft Office and other relevant software programs
