Position: Care Manager
Direct Recruit Agency is currently seeking a highly qualified and experienced Care Manager to join our team on a full-time, permanent basis. Key Responsibilities:
- Oversee the day-to-day operations of our care services, ensuring the delivery of high-quality care to our clients
- Manage a team of care staff, providing guidance, support, and training as needed
- Develop and implement care plans for clients, ensuring they are tailored to their individual needs and regularly reviewed and updated
- Conduct regular assessments of clients' care needs and make necessary adjustments to their care plans
- Maintain accurate and up-to-date records of clients' care and progress
- Liaise with clients, their families, and healthcare professionals to ensure a coordinated approach to care
- Monitor and manage the budget for care services, ensuring efficient use of resources
- Stay up-to-date with industry developments and best practices, implementing changes as needed to improve the quality of care provided
- Adhere to all relevant policies, procedures, and regulations to ensure compliance with legal and ethical standards
- Handle any complaints or concerns from clients or their families in a professional and timely manner
- Participate in recruitment and training of care staff, ensuring a high standard of care is maintained at all times
Qualifications and Skills:
- Master's degree in Social Work, or a related field
- Minimum of 2-5 years of experience in a managerial role in the healthcare industry (Acute, Ambulatory, or Post-Acute) and 2 years experience in Acute Care Management ( Care Coordination)
Willing to travel always
Management experience is required
The position requires a California clinical Social Worker license
- Knowledge of relevant regulations and best practices in the care sector
- Strong leadership and communication skills
- Excellent organizational and time-management abilities
- Compassionate and empathetic approach to care
- Ability to work well under pressure and make sound decisions
- Proficient in Microsoft Office and other relevant software
- Valid driver's license and access to a reliable vehicle
