About the Role:

  1. Casework & Support:

    • Provide one-to-one support to care leavers aged 16-25, helping them plan and manage their transition to independent living.
    • Develop and implement pathway plans tailored to individual needs, ensuring each young person has the necessary resources and support to achieve their goals.
    • Ensure care leavers are actively engaged in their pathway planning and have access to the required services (education, employment, accommodation, and health).
  2. Advice and Guidance:

    • Offer practical advice on a range of issues including education, training, employment, housing, health, and financial independence.
    • Support care leavers with managing their finances, accessing welfare benefits, and budgeting effectively.
    • Assist with job search, educational opportunities, and training placements to improve life chances.
  3. Risk Assessment and Safeguarding:

    • Identify and assess risks to the welfare of young people and take appropriate action to safeguard them, liaising with other professionals as necessary.
    • Advocate on behalf of care leavers, ensuring they receive the support and protection they need to stay safe.
  4. Multi-Agency Working:

    • Work collaboratively with social workers, health professionals, education providers, housing agencies, and other stakeholders to ensure a holistic approach to care leavers needs.
    • Attend multi-disciplinary meetings and ensure that information is shared effectively to provide coordinated support.
  5. Monitoring & Reporting:

    • Regularly review and update pathway plans to reflect the evolving needs of each young person.
    • Maintain accurate and up-to-date records of work with young people, including case notes, assessments, and progress reports, in accordance with council policies and statutory requirements.
  6. Advocacy and Empowerment:

    • Empower young people to take responsibility for their own lives and decisions, offering a supportive and encouraging environment.
    • Assist young people in accessing community resources, networks, and peer support to enhance their independence and resilience.
  7. Specialist Support:

    • Provide support for young people with additional or complex needs, including mental health issues, substance misuse, or those who have experienced trauma.

Key Skills & Experience:

  • Professional Qualification:
    A recognised social work qualification (e.g., Social Work Degree, CQSW, DipSW, or equivalent) and current HCPC registration.
  • Experience:

    • Minimum of 2 years' experience in direct work with young people, particularly care leavers or in similar social care settings.
    • Knowledge and experience of working within the statutory and voluntary sectors.
    • A strong understanding of child protection procedures and safeguarding protocols.
  • Skills:

    • Excellent communication and interpersonal skills, with the ability to build rapport with young people and work collaboratively with other professionals.
    • Strong organisational skills and the ability to manage caseloads effectively.
    • Ability to assess risk, make decisions, and take appropriate actions to safeguard young people.
    • Competence in using case management systems and writing reports.
  • Personal Attributes:

    • Empathy, patience, and a non-judgmental approach to working with vulnerable young people.
    • Commitment to empowering young people and promoting their rights and welfare.
    • Resilience and the ability to remain calm and professional in challenging situations.
    • Motivated, proactive, and able to work under pressure to meet deadlines and objectives.

Qualifications & Requirements:

  • Essential:

    • A recognised social care qualification (e.g., Social Work Degree, CQSW, DipSW).
    • HCPC (Health & Care Professions Council) registration.
    • Significant experience working with young people in the care system or in related services.
    • Knowledge of the Children (Leaving Care) Act 2000 and other relevant legislation.
  • Desirable:

    • Additional qualifications related to working with young people (e.g., youth work qualifications, NVQ in social care).
    • Experience in supporting care leavers through the transition to independent living.
