Type of Patients Served
4GAB is a 20-bed telemetry unit that serves patients requiring medical observation to include cardiac monitoring, hydration, electrolyte repletion, transfusion services, pain management, care coordination, and other services not be readily available in an outpatient setting.
Ages of Patients Served
Adult and geriatric population.
Criteria for Entry:
A. Hospitalist Observation patients with the following exclusions
• Current psychiatric/behavioral/substance abuse issues
• Total nursing care/non-ambulatory
• Need for higher level of care/nursing home placement
B. Low-risk Chest Pain patients admitted to Cardiology
C. Other Outpatients requiring treatment and/or monitoring prior to discharge
• Surgical services
• Oncology patients requiring transfusion/hydration
• Post-interventional radiology
• Post-cardiac catheterization/radiofrequency ablation
Hours of Operation- 4 Gabrilove is operational 24 hours a day, seven days a week.
Methods Used to Assess and Meet Patient Care Needs
Evidence-based practice models will be utilized in the assessment and evaluation of patient care needs. Team members will collaborate to develop, implement, and expedite a holistic care plan.
Staffing
Pattern: Unit is staffed for 20 patients by 4 RNs and 3 PCTs around the clock, with an IA and resource RN assigned weekdays. Census is variable and staff is floated/flexed according to need. It will be the responsibility of the charge RN to determine the appropriate changes, relative to census and acuity, calling on the Nurse Manager/ Supervisor for assistance.

• EKG competency

• Stroke Certification

• Current Basic Life Support
• Cross-train as Information Associate
• EKG technician
• Phlebotomy
Goal of Department/Unit: Expedite quality patient care with a proactive, multidisciplinary approach.
Methods for Communication and Collaboration
A. Internal: Mandatory monthly unit staff meetings are held. An Education Binder is located centrally at the Nurses’ Station with updated protocols and processes; staff is to review and initial weekly at a minimum. There are several communication boards located in the department, one in the break room and others throughout the unit. Email is the preferred method for all staffing/scheduling communication, and Nurse Manager maintains an open door policy.
B. External: Communication with Logistics occurs regularly regarding patient placement and bed availability. Hospitalist providers are present daily on the unit and participate in rounding to discuss patient care plans with Care Coordination and Utilization Management. Other specialties often contribute to these discussions, including but not limited to Cardiology, PT/OT, Neurology, and GI. Unit representatives participate in multidisciplinary hospital based committees. Infection Control, Senior Leadership, EVS, Spiritual Services, Nutrition Services, and Pharmacy round on units regularly. Patient care education is delivered by Nursing Education Department in person by assigned Nurse Educator, online via Healthstream, or relayed to Nurse Manager via email to distribute to staff.
Plan to Improve
• Identification and implementation of unit based standards
• Reporting of events to Quality Improvement and investigation into cause, prevention
• C.A.R.E rounding by all staff
• Continue to monitor LOS and improve process efficiency
Infection Control
4GAB will follow the established infection control polices and procedures as outlined in the infection control manual.
Environmental Safety
4GAB will follow all emergency preparedness plans for the hospital. A disaster call list will be maintained and updated quarterly, located at the main Nurses’ Station.
