As a Clinical Staff Pharmacist Night Lead, you will be an integral part of a multidisciplinary care team, providing expert pharmaceutical care and ensuring medication safety. This position emphasizes antimicrobial stewardship, optimized pharmaceutical dosing, and core clinical activities, including pharmacokinetics and therapeutic interchange.

Signing Bonus: Yes
Bonus Description: $3000 - $5000 Allied Health bonus offered, depending on experience ($3k for less than 3 yrs; $5k for 3 or more years)
Relocation Package: Partial

Key Responsibilities:

  • Interpret and validate medication orders for safety and appropriateness.
  • Perform order verification, IV to PO conversion, and other core clinical activities.
  • Monitor patient drug therapies and provide consultation to medical staff.
  • Assist with medication histories and ensure safe, effective drug administration.
  • Provide leadership and direction to support staff during overnight shifts.
  • In the absence of a supervisor, assume essential leadership responsibilities.

Shift Information:

  • Work onsite as part of a two-pharmacist team during overnight hours.
  • Rotate weekends and holidays as part of the inpatient pharmacy schedule.

Patient Populations Supported:

  • Oncology, surgery, cardiology, pediatrics, adult and pediatric critical care, peri-operative, psychology, same-day infusions, renal, and rehabilitation.

Education and Experience:

  • Education: Doctor of Pharmacy (Pharm.D.) preferred, or Bachelor of Science in Pharmacy from an ACPE-accredited school.
  • Experience:
    • Two years of clinical pharmacist experience in a health system preferred.
    • ASHP-accredited Pharmacy Residency or equivalent experience is preferred.
  • Licensure: Must be a licensed pharmacist in Vermont.

Must-Haves

  1. Doctor of Pharmacy (Pharm.D.) degree or Bachelor of Science in Pharmacy from an ACPE-accredited institution.
  2. Licensed Pharmacist in the State of Vermont.
  3. Familiarity with core clinical activities, such as order verification, pharmacokinetics, and therapeutic interchange.
  4. Ability to work overnight shifts and rotate weekends and holidays.

Nice-To-Haves

  1. ASHP-accredited Pharmacy Residency or equivalent experience.
  2. Two years of clinical pharmacist experience in a health system.
  3. Strong knowledge of medication safety and antimicrobial stewardship.
  4. Experience in oncology, surgery, cardiology, or critical care.
  5. Leadership skills to assume responsibilities in the absence of a supervisor.
