Our client is seeking an experienced Director of Case Management to provide leadership and oversight for case management, discharge planning, and utilization management functions within an acute‑care environment. This role ensures regulatory compliance, supports efficient patient throughput, and drives quality outcomes across the continuum of care.
The ideal candidate is a collaborative, solutions‑oriented leader with strong clinical judgment, operational awareness, and a commitment to patient‑centered care.

Responsibilities:
- Monitor patient flow, care coordination, and resource utilization.
- Integrate evidence‑based practices into case management processes.
- Manage departmental budgets and support internal/external audits.
- Ensure compliance with Medicare, CMS, and regulatory requirements.
- Develop departmental goals, policies, and workflows aligned with organizational standards.
- Collaborate with multidisciplinary teams to optimize care transitions and reduce avoidable delays.
- Oversee staffing, hiring, onboarding, evaluations, and professional development.
- Foster a supportive, growth‑oriented environment for case management staff.
- Lead and manage case management operations, including utilization review and discharge planning.
Qualifications/Must-Haves:
- BLS certification.
- Current RN license in Iowa or multistate compact.
- Graduate of an accredited Registered Nursing program.
- Strong knowledge of Medicare regulations and compliance.
- Excellent communication, organizational, and problem‑solving skills.
- Bachelor's degree required; Masters preferred (Nursing, Social Work, Healthcare Administration, or related field)
- Minimum 2 years of Case Management experience in utilization management, discharge planning, or cost/quality management programs.
Preferred Skills:
- Leadership experience in a hospital or healthcare setting.
- Experience with interdisciplinary collaboration and throughput optimization.
- Ability to work independently while supporting a high‑performing team.
