Job Description:

We are seeking a highly motivated and experienced Home Health LPN to join our team in Canonsburg, PA. In this role, you will work collaboratively with a multidisciplinary team to provide compassionate and comprehensive care to patients in their homes.

Responsibilities:

• Conduct initial and ongoing assessments of patients' health status

• Develop and implement individualized care plans based on patient needs

• Administer medications and treatments as prescribed by the physician

• Monitor and document patient progress and report any changes to the physician

• Educate patients and their families on proper care techniques and disease management

• Collaborate with other healthcare professionals to provide coordinated and holistic care

• Maintain accurate and up-to-date patient records

• Adhere to all state and federal regulations and company policies

Qualifications:

• Current LPN license in the state of Pennsylvania

• Minimum of 1 year of experience in home health or a related field

• Strong communication and interpersonal skills

• Ability to work independently and in a team environment

• Excellent time management and organizational skills

• Compassionate and empathetic attitude towards patients

• Basic computer skills and proficiency in electronic medical records

• Valid driver's license and reliable transportation
