Responsibilities:

  • Obtains and records Vital Signs and weights per policy;
  • Participates in the care planning process and implements care according to care plan;
  • Provide hands on patient/client care;
  • Grooming of patient/client;
  • Uses Standard precautions when caring for all patients. Adheres to contact and or droplet precautions as indicated;
  • Feeds or assists patients with meals and provides additional nourishment and hydration per care plan;
  • Emotional support for patient/client;
  • Participates in quality improvement activities as requested;
  • Collects and bags soiled linen and delivers to dirty linen area;
  • Performs skin care treatments, as permitted by state regulation;
  • Assists physician and or licensed nurse with treatments and procedures as needed;
  • Collaborates and coordinates with other departments to provide timely effective care consistent with individuals needs, choices and preferences;
  • Cares for personal belongings;
  • Reports changes in patients condition, patient/family concerns or complaints to charge nurse and or supervisor;
  • Toileting and incontinence care for patient/client.

Requirements:

  • Reliable transportation;
  • TB shot;
  • Active NH LNA license;
  • Have reliable transportation and meet Agency requirements for vehicle insurance;
  • Physical exam within the last year;
  • Must be licensed or license eligible in VT and NH;
  • Automobile insurance
