Position Title: Registered Nurse

Work Set Up: Onsite

Schedule: 9:00 PM - 6:00 AM PHT

Job Location: Clark Outsourcing - Building 35 Philexcel Business Park, Clark Pampanga

Quick Rundown:

Responsible for managing Home Health referrals, verifying eligibility, and coordinating smooth patient transitions. Perform admission reviews, obtain authorizations, and assist patients and families with service details and documentation. Communicate updates to stakeholders, monitor performance, and ensure quality care. Serve as a liaison to resolve customer issues and provide after-hours support as needed.

What You'll Do:

• Perform admission reviews to determine care levels and readiness for discharge

• Manage referrals, verify eligibility, and ensure smooth transitions to Home Health

• Assist patients, families, and referral sources with service details, insurance, and documentation

• Obtain admission authorizations and coordinate intake processes

• Communicate referral updates to marketers, clinicians, and coordinators

• Monitor referrals, staff performance, and service utilization to maintain quality care

• Handle office communications, including after-hours on-call support

• Act as a liaison between partners and company departments, resolving customer issues

What We're Looking For:

• Must be a Registered Nurse

• Strong customer service, organizational, and communication skills

• Proficient in computer use, including Microsoft Word (Excel preferred)

• Ability to multitask, prioritize, and work independently

• Familiarity with Home Health processes and procedures is a plus
