Position Title: Registered Nurse

Reports To: Partner, Partner Advisor

Primary Relationships: Partner Team

Schedule: 9:00 PM - 6:00 AM PHT


Responsibilities:

• Patient Assessment:
  • Conduct comprehensive assessments of patient needs and medical history
  • Collaborate with healthcare professionals to determine the most suitable care plan for patients
    
• Care Coordination:
  • Facilitate the seamless transition of patients into our healthcare programs
  • Coordinate with physicians, caregivers, and other healthcare providers to ensure continuity of care
    
• Documentation and Recordkeeping:
  • Maintain accurate and up-to-date records of patient orders and care plans
  • Ensure compliance with healthcare documentation standards and regulatory requirements
    
• Communication:
  • Communicate effectively with patients, families, and healthcare team members
  • Provide clear and concise information regarding healthcare programs and services
    
• Patient Education:
  • Educate patients and their families on healthcare programs, treatment plans, and available resources
  • Ensure patients have a clear understanding of their care plans and are equipped to participate in their healthcare journey
    
• Quality Assurance:
  • Monitor and evaluate the effectiveness of care plans and interventions
  • Collaborate with the quality assurance team to identify areas for improvement and implement corrective actions
    
    

Qualifications:

• Bachelor of Science in Nursing (BSN) preferred
• RN license
• Minimum of 5 years of nursing experience, preferably in a clinical or community healthcare setting
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Knowledge of healthcare regulations and documentation standards
