Quick Rundown:
We are looking for a dedicated Customer Service Manager (Healthcare) to join our team. This role involves coordinating patient care, collaborating with healthcare teams, and ensuring that patients receive comprehensive support.
What You'll Do:
Serve as the primary case manager for assigned patients, developing individualized care plans
Coordinate all aspects of patient care, including home health services, physician appointments, and community resources
Educate patients and families on managing chronic illnesses and utilizing available resources
Act as an advocate for patients and families, providing guidance throughout the healthcare process
Identify potential barriers to care and develop solutions to improve patient outcomes
Maintain accurate and up-to-date documentation in patient records
Participate in quality improvement initiatives to enhance care delivery
Identify discharge planning needs and implement plans before patient discharge.
What We're Looking For:
Minimum 3 years of experience in nursing, with a focus on home health care
At least 1 year of experience with Data Soft Logic (DSL) EMR System
Bachelor’s degree in nursing or a related field
Active RN license in good standing
Case management certification (preferred)
Strong clinical assessment and communication skills
Ability to work independently and as part of a team
