Job Title: Medical Social Worker (MSW)

Department: Clinical Services

Reports To: Director of Clinical Services/Clinical Manager

Job Type: Full-time/Part-time (as applicable)

Job Summary: A Medical Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence. This includes implementing physician orders and plans of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Services may be furnished by a qualified social worker or by a social work assistant under supervision.

Qualifications:

  • Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education with one year of experience in a healthcare setting.
  • Bachelors degree in social work, psychology, sociology, or related field, plus one year of social work experience in a healthcare setting (functions as an assistant).
  • Minimum of two years of relevant experience (for assistant role).
  • Preferably one year of experience in home care.
  • Acceptance of the agencys philosophy and goals.
  • Ability to exercise initiative and independent judgment.
  • Capacity to work collaboratively towards common goals.

Responsibilities:

  • Adhere to agency policies and procedures and promote Quality Assurance and Performance Improvement (QAPI) principles.
  • Serve as a consultant to agency personnel and participate in community resource coordination.
  • Maintain confidentiality and safeguard all patient information.
  • Participate in patient care conferences, in-services, continuing education programs, and discharge planning.
  • Develop and coordinate all social services within the agency.
  • Document all patient/family services as required by agency policy.

Working Environment:

  • Works indoors in agency office and patient homes.
  • Travels to/from patient homes.

Job Relationships:

  • Supervised by the Director of Clinical Services/Clinical Manager.
  • May supervise Social Work Assistants if applicable.

Risk Exposure:

  • High risk

Lifting Requirements:

  • Ability to engage in physical activities including bending, lifting, and standing as necessary.
  • Light lifting may be required.

Performance Appraisal/Evaluation:

  • Evaluated based on documentation quality, community resource utilization, patient care involvement, adherence to agency policies, and professional development activities.
