Job Overview:
A growing geriatric healthcare team is seeking a Licensed Nurse Practitioner (NP) to deliver internal medicine services in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). This role offers a stable, rewarding clinical leadership opportunity focused on long-term and post-acute care. As the facilitys on-site provider, you will play a vital role in improving patient outcomes and minimizing avoidable hospitalizations.

Key Responsibilities:

Perform comprehensive patient assessments, diagnoses, and develop treatment plans

Conduct monthly stability checks and address acute conditions

Prescribe medications and treatments per state guidelines

Collaborate with nursing staff, attending physicians, and specialists

Engage in family communication and care planning discussions

Maintain clear, accurate documentation with appropriate CPT coding

Lead or participate in patient care conferences and advanced care planning
-
Promote continuity of care across facility teams
Qualifications:

Active APRN license in Kentucky (Required)

Current DEA license (Required)

At least 1 year of experience in SNF, ALF, or LTC settings (Preferred)

Strong assessment and clinical documentation skills

Excellent interpersonal and communication abilities

Ability to operate independently while remaining team-oriented
Compensation & Work Structure:

Minimum 2 days per week (Your choice of days)

Additional shifts available based on your availability

All patients seen at a single location within the Paducah area
