Position Overview
We are seeking a highly organized and proactive Patient Advocate / Case Coordinator to join our team. In this role, you will serve as the primary liaison between patients, their legal representation, and medical professionals, to ensure seamless coordination of patient care throughout treatment.
The ideal candidate has exceptional attention to detail and documentation skills, communicates professionally, successfully manages multiple tasks at once, and thrives in a fast-paced environment.
This position plays a critical role in supporting patient care, coordinating treatment-related services, and ensuring routine, timely communication with referral sources and legal representatives.
Responsibilities
Patient & Case Coordination
- Coordinate patient care and treatment-related services, including routine appointments and procedures
- Maintain ongoing communication with patients regarding appointments, procedures, and treatment plans
- Identify and assist with resolving barriers to patient care
- Complete and manage outgoing orders and referrals
Legal & Provider Communication
- Communicate regularly with patient legal representation, case managers, and referral sources regarding patient care and treatment status
- Provide routine, timely treatment updates through medical record and billing management
- Complete the orders, referrals, and procedures workflow promptly to ensure no gaps in treatment
Administrative Responsibilities
- Maintain accurate, detailed documentation in real time and in all required systems
- Manage high-volume email and phone communications and task follow-up
- Maintain organized case tracking and workflow management
Qualifications
- Bilingual in English & Spanish preferred
- Strong organizational, multitasking, and task prioritization skills
- Excellent written and verbal communication skills
- Strong attention to detail, documentation, and follow through
- Ability to work independently and exercise sound judgement
- Proficiency in Microsoft Office Suite, Adobe Acrobat Pro, and related apps
- Experience with EMR/EHR systems
- Experience with Smartsheet or other similar management platforms preferred
Education & Training
- College Graduate of any four year course.
- Minimum one year of relevant healthcare administration experience required
- Minimum one year of relevant personal injury experience required
- HIPAA compliance training required (training available if necessary)
