  • Job Description 
  • Responsibilities:

    • Collaborate with the interdisciplinary team to assess, diagnose, and formulate treatment plans for residents with acute or exacerbating medical conditions.
    • Conduct comprehensive physical assessments and review medical histories to determine the appropriate course of treatment for residents.
    • Order and interpret diagnostic tests, including laboratory work, imaging studies, and other relevant tests to aid in diagnosis and treatment planning.
    • Provide timely and evidence-based medical interventions, including medication administration, wound care, IV therapy, and other treatments as needed.
    • Manage and monitor residents in critical or unstable conditions, ensuring appropriate interventions are taken promptly.
    • Work closely with attending physicians, consulting specialists, and other healthcare professionals to ensure seamless coordination of care and communication.
    • Educate residents and their families about treatment options, disease management, and preventive care strategies.
    • Document all assessments, interventions, and outcomes accurately and promptly in the electronic medical records system.
    • Participate in regular multidisciplinary team meetings to discuss resident progress, care plans, and any necessary adjustments to treatment strategies.
    • Contribute to quality improvement initiatives by actively participating in performance improvement projects and promoting evidence-based practice.
    • Provide guidance and mentorship to nursing staff on best practices for acute care management, fostering a culture of learning and professional growth.

    Qualifications:

    • Master's or Doctorate degree in Nursing with a specialization in Acute Care Nurse Practitioner.
    • Current state Advanced Practice Registered Nurse (APRN) license and Acute Care Nurse Practitioner certification.
    • Proven experience as an Acute Care Nurse Practitioner, preferably in a long-term care or nursing home setting.
    • Strong clinical assessment and diagnostic skills, with the ability to make informed decisions in high-pressure situations.
    • Excellent communication skills to effectively interact with residents, families, colleagues, and other healthcare professionals.
    • Proficiency in using electronic medical records (EMR) systems for documentation and communication.
    • Ability to work collaboratively in a team-based healthcare environment.
    • Dedication to maintaining up-to-date knowledge of best practices, medical advancements, and regulations in acute care.
