The Inpatient Rehab Nurse Practitioner (NP) will provide comprehensive care to patients undergoing rehabilitation after a major illness or injury. The NP will collaborate with a team of healthcare professionals to assess, diagnose, and develop treatment plans aimed at optimizing recovery and enhancing the quality of life for patients in an inpatient rehab setting.
Key Responsibilities:
- Patient Assessment:
- Conduct thorough physical examinations and health assessments on patients admitted for rehabilitation.
- Review patients medical history, medications, and rehabilitation progress.
- Diagnosis and Treatment:
- Develop and implement individualized treatment plans in collaboration with physicians and rehabilitation therapists.
- Monitor patients progress and adjust treatment plans as needed.
- Patient Education:
- Provide education to patients and families regarding rehabilitation processes, treatment goals, and home healthcare instructions.
- Counsel patients on lifestyle modifications and self-management strategies post-discharge.
- Collaboration and Coordination:
- Work closely with a multidisciplinary team, including physicians, physical therapists, occupational therapists, and speech therapists, to enhance patient care.
- Participate in interdisciplinary team meetings to discuss patient progress and discharge planning.
- Documentation:
- Maintain accurate and up-to-date patient records in compliance with regulations and facility policies.
- Document assessment findings, diagnostic tests, treatment plans, and patient progress in electronic health records.
- Quality Improvement:
- Participate in quality improvement initiatives and audits to ensure high standards of patient care.
- Stay informed about best practices in rehabilitation and integrate evidence-based practices into care.
- Emergency Response:
- Respond to acute changes in patient conditions, providing prompt medical interventions as necessary.
Qualifications:
- Education: Masters degree in Nursing from an accredited program.
- Licensure: Current Nurse Practitioner licensure in [State], with Board Certification as a Nurse Practitioner.
- Skills:
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Proficient in EMR documentation and electronic health record management.
- Ability to work collaboratively within a multidisciplinary team.
Additional Requirements:
- Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) certification preferred.
- Commitment to continuous education and professional development in rehabilitation nursing.
