Employment Application Declaration and Consent

I hereby declare that the above information given by me is true and correct. I also agree that my references can be contacted for reference purposes. 

By submitting this Employment Application, I authorize and fully consent Oakwood Worldwide (Asia) Pte Ltd or its Oakwood affiliated subsidiaries, to collect, retain, use and disclose my personal data, as provided in this employment application form, or (if applicable) obtained by the Company as a result of my job application, for the following purposes in accordance with the Personal Data Protection Act 2012 in Singapore, and where appilcable Privacy Data Laws apply. 

The purposes of collection includes:

(a)       The processing of this job application; and/or

(b)       After the acceptance of the Company's job offer, to use my personal information and data for the administration listed below:

• Monthly Payroll;
• Pre-Employment Medical Checkup (if any);
• Application/Renewal of Foreign Visas or Work Permit/Visas or Work pass (if any);
• Applicable Pension Contribution or equivalent, CPF contributions (if any); and/or
• Local Tax Reporting purposes (if any).

I understand that if I should withdraw my consent, I shall comply with the “Withdrawal of Consent” as indicated in Section 16 of the Personal Data Protection Act 2012 and I shall inform the Privacy Officer of Oakwood Worldwide (Asia) Pte Ltd or its Oakwood affiliated subsidiaries if a withdrawal is needed.
