My client is a leader in providing high-quality healthcare services. They are seeking a skilled and compassionate Case Manager to join the team in the Manchester area. This role involves coordinating comprehensive care for patients, ensuring they receive the best possible outcomes.

Key Responsibilities:

To take joint ownership of a caseload of complex clients and manage their care package holistically in collaboration with the clients appointed Nurse Case Manager.

• To develop, review and implement client care plans and risk assessments in line with CQC regulations and best practice guidance.
• To co-ordinate and lead the transition (start-up) and ongoing continuity of all allocated complex care packages..
• To engage in appropriate networking and to work proactively in partnership with Clinical Commissioning Groups and Social Services, attending case conferences/ MDT meetings as required.

• To ensure complaints are managed in line with internal organizational policies

Qualifications:

• Valid license in the UK.
• Previous experience in case management or a related clinical role.
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of healthcare regulations and best practices.
• Knowledge of CQC regulations and quality assurance processes
