Key Responsibilities:

• Coordinate and manage comprehensive care services for patients with complex health needs.
• Develop and implement individualized care plans.
• Lead and support a multidisciplinary team of healthcare professionals.
• Ensure compliance with all relevant healthcare regulations and quality standards.
• Advocate for patients and their families, providing education and support.
• Manage resources efficiently to ensure effective service delivery.

Qualifications:

• Level 5 or equivalent in Health and Social Care
• Experience in managing care for individuals with complex health needs.
• Strong leadership, communication, and organizational skills.
• Knowledge of CQC regulations and quality assurance processes.
