Responsibilities:
- Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life
- On-going individual and family counselling through face-to-face sessions or home visitations in order to help navigate the personal and ethical dilemma, cultural conflict and issue relating to religion, spirituality and the meaning of life
- Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve optimal end of life goals
- Engage families involvement and encourage their visits, as well as prepares patients / families for the end-of-life stage and managing the life closure and the dying process
- Provide bereavement information, education and support to help the families of the deceased patients cope with their grief and loss
- Attend Interdisciplinary Patient Care Conferences with medical, nursing, allied health colleagues to discuss plans for total management of the patient
- Counsel patients and their family members on financial and discharge care plans
- Perform field work involving visiting patients in hospital wards and other social agencies to help patients alleviate or resolve their medico-social problems
- Prepare social and other related reports for management and other related institutions / agencies
- Present cases for discussion and engages actively on formal platforms such as departmental or inter-departmental meetings for the purpose of sharing, reviewing and clarifying of atypical, difficult issues
- Maintain constant networking with MSW, Counsellor, staff nurses, nursing officers and relevant medical staff / authorities of other organisations
- Undertake more workload or difficult cases when required
- Orientate and On-Job-Trains new staff and / or attached interns on their performance of duties
- Participate in in-service education programme and follow-up to ensure optimum transfer of learning
- Present at Journal club the works and development in palliative and end of life care setting of PSS to other disciplines in the hospice
- Document all social interventions for record, within the expected time frame set by the department
Requirements:
- Degree in Social Work and / or Master in Counselling preferably.
- Registered with the Singapore Association of Social Workers for Social Workers or Singapore Association for Counselling (SAC).
- At least 3 to 9 years of relevant experience in a healthcare environment preferably.
- Bilingual and ability to communicate in local dialects preferred.
