Rate of Pay: £17.50- £22.41 per hour

Summary:

The Social Care Facilitator is responsible for supporting vulnerable adults and their carers by contributing to assessment, care planning, and care management activities within a multidisciplinary adult social care team. The role involves carrying out assessments, supporting care management processes, facilitating access to community resources and services, and assisting with the provision of assistive equipment where appropriate. The Social Care Facilitator plays an important role in enabling individuals to remain as independent as possible while ensuring compliance with relevant legislation, safeguarding procedures, and council policies.

Responsibilities:

• Contribute to the assessment of the needs of vulnerable adults and their carers in accordance with the Care Act 2014 and council procedures.
• Undertake allocated care management responsibilities for individuals within agreed levels of responsibility.
• Provide information, advice, and guidance regarding available services, community resources, and support options.
• Support integrated working across health, social care, and community services.
• Identify the need for specialist equipment or home adaptations and refer cases to Occupational Therapy or other specialist services for assessment.
• Identify and manage risks while balancing individuals' rights, independence, and safety.

Qualifications:

• NVQ Level 3 in Health and Social Care, Care Certificate, or equivalent relevant qualification, or willingness to work towards one.
• GCSEs (or equivalent), including English and Mathematics, or equivalent experience.

Essentials:

• Experience supporting vulnerable adults within a health, social care, or community setting.
• Experience working with individuals with a range of physical, mental health, learning disability, or age-related support needs.
• Experience contributing to assessments, care planning, or care management activities.
