Outreach Family Support Worker

 Job Description:

We are working closely alongside a Local Authority in Liverpool to assist with the appointment of an Outreach Family Support Worker, on a 3-month contract, likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £15.97 - £20.98 per Hour

Summary:

The Liverpool Early Help service adopts a strength-based, family-centered approach to support children, young people, and families in overcoming emerging difficulties and improving long-term outcomes. As an Outreach Family Support Worker, you will act as the lead professional for allocated families, ensuring coordinated support through multi-agency collaboration and a whole-family approach.

Responsibilities:

  • Undertake Early Help Assessments to identify family strengths and areas for support.
  • Develop and maintain tailored Support Plans in partnership with families and relevant agencies.
  • Act as the Lead Professional, chairing Team Around the Family (TAF) meetings to monitor progress and adapt plans as needed.
  • Engage in direct work with children and young people to explore their experiences, views, and worries, ensuring their voices shape the support provided.
  • Liaise and coordinate with multi-agency professionals to ensure seamless service delivery and effective support networks for families.

Essential Criteria:

  • Experience working directly with children, young people, and families in a support or intervention-based role.
  • Knowledge of strength-based approaches and early help assessment frameworks.
  • Strong partnership and collaboration skills, particularly in multi-agency settings.
  • Ability to plan, chair, and review meetings with professionals and families.

  • This role requires an Enhanced DBS
