We are working closely alongside a Local Authority in Cardiff to assist with the appointment of a Social Work Assistant, on a 3-month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £15.44 - £19.31 per Hour

Responsibilities:

• To arrange and monitor appropriate services in line with individual care plans.
• To provide advice and practical help.
• To liaise with voluntary and statutory agencies.
• To complete annual care plan reviews and or reassessments.
• To participate in team meetings and duty rotas.

Qualifications:

• Experience of working with older people and adults with a physical disability through direct contact in a working environment.
• Ability to work independently and as part of a team.
• Ability to make relationships with se5rvice users and carers.

• You should be available to work immediately or at a short notice.
• You should have right to work in U.K
