We are working closely alongside a Local Authority in Westminster to assist with the appointment of an Occupational Therapist Care Manager, on a 3-month contract, likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £23.12 - £31.52 per Hour

Responsibilities:

• Deliver skilled Occupational Therapy interventions to residents in their homes.
• Manage cases for up to six weeks with a goal-focused reablement plan.
• Conduct holistic assessments to identify clinical and personal needs.
• Develop and implement intervention plans, including complex movement and handling assessments.
• Order and provide adaptive equipment to support independent living.

Requirements:

• HCPC-registered Occupational Therapist.
• Experience in adult social care or community settings.
  Experience in complex movement and handling assessments and care planning

Please note:

• You should be available to work immediately or at a short notice.
• You should have right to work in U.K
• This role requires an enhanced DBS
