Service Manager - Integrated Hospital Discharge

 Job Description:

We are working closely alongside a Local Authority in Liverpool to assist with the appointment of a Service Manager - Integrated Hospital Discharge, on a 1-year contract, likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £356.21 - £450.00 per day

Responsibilities:

  • The post holder will provide high level advice, guidance and support in
    specialist areas (e.g., No Recourse to Public Funds & Prevent), to aid decision making at all levels.
  • To guide, shape and implement programmes of change and development to improve peoples experience, through remodelled collaborating with services providers and community organisations.
  • To measure, analyse and report on service performance using data to
    establish trends and future service provision.
  • To implement accountability and responsibility frameworks to ensure decision making and accountability for performance is delegated appropriately.
  • To work with the Councils corporate commissioning, procurement and
    contracting teams in partnership to deliver the best care possible for people to achieve their outcomes and service expectations of the diverse

Qualifications:

  • Educated to degree level or equivalent 
  • Social Work or equivalent professional qualification and registration with a relevant regulatory body 
  • Demonstrable continuous professional development 
  • Significant knowledge of the legislative framework governing adult social care.

Experience:

  • Demonstrated operational service delivery management expertise 
  • Experience of budget management 
  • Experience in developing, implementing strategies and programmes including the development of business and delivery plans 
  • Demonstrable track record of change management leading to improved
    outcomes 
  • Demonstrable experience in working within a complex set of multi-agency partnerships
