Assessment and Enablement Officer
Job Description:
We are working closely with a local authority in Barnet to assist with the appointment of an Assessment and Enablement Officer, on a 3-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Rate of Pay: £22.60- £29.81 per hour
Summary:
The Assessment and Enablement Officer will play a key role within Adult Social Care in supporting adults to achieve greater independence, wellbeing, choice and control. The postholder will undertake strengths-based assessments and reviews of care and support arrangements, working with adults, carers, families, providers and partner agencies to ensure that support remains appropriate, person-centred, outcome-focused and proportionate.
Responsibilities:
- Undertake proportionate, person-centred and strengths-based assessments and reviews of adult social care needs and support arrangements.
- Review existing care and support packages to determine whether eligible needs continue to be met and whether agreed outcomes are being achieved.
- Identify and assess changes in an individual's needs, circumstances, risks, strengths, capabilities and wellbeing.
- Ensure that reviews focus on outcomes that are important to the individual rather than solely on the services being received.
Qualifications:
- Relevant qualification or substantial equivalent experience in adult social care, health, social care, community support or a related field.
- Knowledge and understanding of the Care Act 2014 and its application to adult social care.
- Relevant safeguarding adults training and understanding of Making Safeguarding Personal.
Essentials:
- Significant experience of working with adults within a social care, health, community or related support environment.
- Experience of undertaking or contributing to assessments and reviews of adult social care needs and support arrangements.
- Experience of working across a range of care and support settings, such as supported living, residential care, home care, day opportunities and community services.

- You should have right to work in U.K
- This role requires an Enhanced DBS
