Job Description:
We are working closely with Wirral MBC to assist with the appointment of a Placement & Brokerage Officer, on a 3-month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Rate of Pay: £13.78 - £17.73 per hour
Summary:
We are seeking a Placement & Brokerage Officer to join the Care Arrangement Team, responsible for coordinating and arranging care packages for adults with assessed care needs. The successful candidate will work closely with care providers, social workers and health professionals to source appropriate care, monitor provider capacity, and ensure timely, person-centred care arrangements in line with the Care Act 2014.
Responsibilities:
- Coordinate and broker care packages, ensuring accurate recording on case management systems and compliance with organisational standards.
- Liaise with care providers, social workers and health professionals to source appropriate care and facilitate timely service delivery, including hospital discharges.
- Monitor provider capacity, identify market pressures and escalate issues affecting service availability.
- Undertake quality assurance checks on brokerage referrals and ensure compliance with contractual requirements and agreed rates.
- Maintain strong relationships with providers and stakeholders while supporting service improvement through accurate reporting and data analysis.
Qualifications:
- GCSEs (or equivalent), including English and Maths.
Essentials:
- Experience working within an Adult Social Care environment.
- Strong verbal and written communication skills.
- Ability to manage competing priorities and meet deadlines.
- Experience using digital systems for case management, communication and reporting.
- Good understanding of care brokerage processes and working with care providers.
- Commitment to confidentiality, equality and continuous improvement.
- Ability to work as part of a team delivering services 7 days a week between 8am–8pm.
