Physical Therapist (PT) – Outpatient

Overview

We are seeking a licensed Physical Therapist (PT) to join a patient-focused outpatient rehab team in Cheraw, SC. This role involves evaluating, planning, and delivering personalized treatment programs to help patients restore mobility, reduce pain, and improve functional independence.

Key Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans
• Deliver evidence-based physical therapy interventions in an outpatient setting
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and families on recovery plans, exercises, and prevention strategies
• Document patient care accurately and in a timely manner in compliance with clinical standards
• Collaborate with interdisciplinary healthcare professionals to ensure optimal patient outcomes

Required Qualifications

• Graduate of an accredited Physical Therapy program
• Current, active, and unrestricted Physical Therapy license in South Carolina
• Current CPR certification

Preferred Qualifications

• Previous experience in a rehabilitation or outpatient therapy setting preferred
• Strong communication and patient engagement skills
• Ability to work independently while contributing to a team-based care environment
