Heartwood Lodge is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team. In this full-time night shift role, you will provide exceptional nursing care to patients while working closely with a supportive and collaborative multidisciplinary team. This position offers the opportunity to make a meaningful impact in a patient-centered environment.

Schedule

  • Shift: 6:45 PM 7:15 AM
  • Days: Monday,  Friday
  • Hours per Week: 36

Key Responsibilities

  • Assist in patient assessment, evaluation, and documentation of physical, emotional, and social needs.
  • Support the development and implementation of individualized care plans and discharge plans.
  • Provide patient and family education based on established care plans.
  • Adapt to new processes and technologies, maintaining professionalism under pressure.
  • Communicate patient updates and changes effectively with RNs and care team members.
  • Participate in department meetings, in-services, and training opportunities.
  • Safeguard resources while maintaining productivity and supporting staffing needs across units.
  • Contribute to process improvement initiatives to enhance patient outcomes and satisfaction.
  • Document patient information accurately in the medical record system.
  • Delegate tasks appropriately within the scope of practice.
  • Maintain HIPAA compliance and patient confidentiality at all times.
  • Deliver age-appropriate care, adapting procedures and treatments as needed.
  • Uphold the mission, values, and standards of MercyOne North Iowa and the Sisters of Mercy.
  • Perform additional duties as assigned by supervisors or management.

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the state of Michigan.
  • Minimum 1 year of nursing experience required (travel LPN experience a plus).
  • Strong communication, critical thinking, and organizational skills.
