About the job LVN/LPN - Long Term Care (Nights)
Under the direction of the RN Supervisor, the LPN Wing Supervisor carries out the medical plan of care and oversees the Nursing Assistants' care of residents and High School Students to insure that resident care is carried out in accordance with good nursing practice and applicable State and Federal regulations. Client's ideal candidate would be someone who has a passion to work with and interact with residents daily.
GENERAL DUTIES
- Physician's Computer Company: EHR/EMR
- Administers and records medication, treatments and monitoring e.g. glucose checks, as ordered and reports and documents resident response
- Notifies the RN Supervisor immediately of a significant change in residents condition
- Reports and records changes in resident condition to the RN Supervisor
- Communicates resident transfers to immediate family and via email to department heads
- On resident death, supervises and assists with post mortem procedures. Assists with collection of resident's clothing and belongings. Notifies all department heads of death via email, makes phone calls as directed by RN Supervisor
- Submits maintenance requests as needed
- Supervises and directs resident care by nursing assistants to insure the prescribed plan of care is carried out
- Performs additional duties as directed by supervisor
- Assures that staff is familiar with changes in policies and procedures
- Updates the daily census as needed
- Monitors NA documentation for completeness and compliance with regulations

ABOUT THE FACILITY
The Lutheran Home at Kane is a CCRC (Continuum of Care Retirement Community). We provide a wide range of services for the elderly to assist with the aging process. We can provide independent living in our cottages, assisted living in our personal care facility, and skilled nursing care for short-term rehabilitation or long-term stays in our nursing home. Transitioning from one level of care to another is very easy since all of these facilities are located at our 100 High Point Drive location.
