We offer a variety of programs including residential support, day programs, employment assistance, early intervention, and behavior management.

Job Description: We are seeking a dedicated Qualified Developmental Disability Professional (QDDP) to join our team. In this role, you will participate in the Person-Centered Planning Process, conduct interviews and meetings, complete progress notes and assessments, develop training objectives, and ensure quality care for the individuals we serve.

Key Responsibilities:

  • Person-Centered Planning: Participate in the Person-Centered Planning Process and conduct pre-meetings and POM interviews.
  • Meetings and Documentation: Participate in person-centered meetings, special team meetings, and complete monthly progress notes.
  • Assessments: Complete annual assessments, consents, and releases.
  • Training Objectives: Develop quality day and/or residential training objectives for individuals served.
  • Documentation: File PCPs and supporting documents in the person-served residential binder.
  • Observation: Observe staff training with individuals served.
  • Additional Duties: Perform other duties as assigned by the supervisor.

Qualifications:

  • Education: Bachelor’s degree from an accredited college/university in a Human Services related field required.
  • Experience: Experience working with individuals with intellectual disabilities preferred.
  • Skills: Knowledge of mental health terminology, behavioral challenges, and diagnoses impacting the IDD population. Proficient in Microsoft Outlook, Excel, and Word. Clinical documentation experience.
  • Communication: Excellent written and verbal communication skills.
  • Requirements: Ability to pass pre-employment screening (drug, background, MVR).
