SPECIALIST DEMENTIA CARE MANAGER

Role Purpose

The Specialist Care Dementia Manager position is responsible to plan and coordinate person-centred care to support residents to participate in meaningful engagement and supported care opportunities, in the 8-bed Specialist Dementia Care Unit, and 11-bed Memory Support unit.

To oversee and manage the Specialist Dementia Care Unit and Memory Support Unit through coordination of in-reach multidisciplinary services to meet resident needs and outcome goals.

To develop tailored care plans and behaviour support plans to ensure a high standard of care quality for the resident that is in line with expectations for a specialised service delivery model, in conjunction with specialist multidisciplinary partners.

To lead by example to develop a skilled workforce and care environment which achieves de-escalation of very severe (Tier 6) responsive behaviours in the SDCU, and moderate to severe (Tier 4-5) behaviours in the Memory Support unit.

Role Accountabilities

Leadership

o Promote values that reflect the spirit and purpose of employer.

o Educate and lead colleagues to demonstrate the SDCP and Memory Support units strong focus on ongoing care assessment, care planning, and access to therapeutic and meaningful activities for unit residents.

o Provide leadership and support for the wider team to ensure appropriate dementia care is provided for Residents.

o Proactively develop relationships with in-reach specialist team members and referral partners to clearly promote the objectives of the SDCU.

o Provide a supportive approach to communicating with the residents and their representatives, while respecting confidentiality.

o Demonstrate emotional maturity, cultural awareness, and flexibility when dealing with individuals and situations.

o Lead by example, and influence others to create and maintain a supportive culture and calm environment.

o Facilitate engagement opportunities for residents living with dementia, while promoting their personal decision making, and optimum self-direction.

ESSENTIAL

Technical Qualifications

  • AHPRA registered Degree qualifying as a Registered Nursing
  • Level 3 Registered Nurse with greater than 4 years of experience (preferred.) We will consider a qualified Level 2 candidate.

Knowledge, Skills and Experience

o Previous management experience in an acute or aged care setting working with people living with dementia.

o Proven ability to foster and deliver a person-centred approach to care.

o Demonstrated well-developed verbal and written communication skills and well-developed interpersonal skills with a broad range of people, including people from culturally and linguistically diverse backgrounds and people with limited verbal communication capacity.

o Ability to problem solve and deliver flexible care through respectful negotiation with a person living with dementia.

o Strong knowledge and understanding of the Aged Care Quality Standards and how these practically apply in the workplace.

o Demonstrated ability to work collaboratively within a team environment, with strong time management skills.

o Knowledge of Serious Incident Response Scheme (SIRS) and mandatory reporting requirements 

o Experience working with software programs and handheld devices e.g., Word, email, smartphone, iPad tablets etc

o Ability to work autonomously or as part of a team.

o An understanding and skill in appropriate responsive behaviour approaches to care, and a willingness to undertake ongoing training in this specialised area of care.

o A willingness to drive/ travel to referral locations within Tasmania to assess resident suitability for unit admission following eligibility has been determined.

DESIRABLE

  • Graduate Certificate, Diploma or Bachelor in Dementia Care
  • Postgraduate Gerontology qualification
  • Experience working in an aged care, dementia care environments in any healthcare setting

Licenses/Other Requirements

o Provision of a National Police certificate that meets the suitability requirements for unsupervised work in accordance with the Aged Care Act.

o A current drivers license.

o A current registration to work with vulnerable people, with NDIS endorsement.

o Right-to-work certification or Australian Resident o Current annual influenza vaccination 

o A minimum of 3 Covid-19 vaccinations
