JOB DESCRIPTION:
PHYSICIAN - OBGYN HOSPITALIST DALLAS, TX
OB/GYN Hospitalist needed in TX
- Level IV NICU
- High risk obstetrics training and experience required
- Delivery room
- Triage
- GYN ED
- Full-spectrum OBGYN Hospitalist work
SUBMISSION REQUIREMENTS
- Must have active TX Medical License and DEA
- Must have clean background and malpractice
AS A REMINDER:
- All candidates must have met the minimum shift availability listed on the job.
- CV must have all dates in MM/YY to MM/YY format
- Work History must be in Sequential order with most recent on top
- Any Gaps over 30 days must be explained on the CV
- EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
- All Licenses, Boards and Certs with expiration dates must be listed on CV
- Anyone with malpractice must provide a NPDB
- Anyone with legal issues must provide supporting documents
- Availability
- 1st possible date they would be available?
- How many shifts would they be able for per month?
- What month is their schedule completely open?
- Any shift preferences (Day, Night, Blocksetc)?
- Any dates in the next 3-6 months they cant work?
- Any Holidays in the next 3-6 months they would be open to work.?
- If provider does not match to all requirements on the Job Order, you must request permission from MSP Coordinator prior to submitting a Name Clear submission.
- The attached ICA must be used for any provider that is approved for credentialing. This version is also in Trio under your Name & then by clicking Help. Please do not utilize any past versions.
