JOB-1 DESCRIPTION:

One of the hospitals is looking for Emergency Medicine in Sebring, FL

SUBMISSION REQUIREMENTS

• Must have Medical License and DEA at time of Name Clear
• Must have clean malpractice/background for temp privs
• Must be Board Certified EM
• Must send self-query NPDB and EPA with presentation
• Must be able to work at least 7 shift per month
• Must have MM/YYYY format on CV and gaps be explained
• Must have all licenses and certifications listed on CV
• Must have current start date and availability

AVAILABILITY

• 1st possible date they would be available?
• How many shifts would they be able for per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc)?
• Any dates in the next 3-6 months they cant work?
• Any Holidays in the next 3-6 months they would be open to work.?

Coverage:

• Main ED 88 hours,
• Peds 24 hours, and
• Mid-Level 59 hours
• Main ED Physician Hours: 5a-1p; 7a-7p; 9a9p; 11a9p; 12p-2a; 5p-5a; 7p-7a; 10p-8a
