JOB DESCRIPTION:

PHYSICIAN - OBGYN HOSPITALIST PLANO, TX

Open need for OB/GYN Hospitalist

Standard OB Hospitalist work

SUBMISSION REQUIREMENTS

  • Must have active TX Medical License and DEA
  • Must have clean background and malpractice

Clinicians must have the below reflected on their case logs in the last 2 years in order to be granted privs

AS A REMINDER:

  • All candidates must have meet the minimum shift availability listed on the job.
  • CV must have all dates in MM/YY to MM/YY format
  • Work History must be in Sequential order with most recent on top
  • Any Gaps over 30 days must be explained on the CV
  • EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
  • All Licenses, Boards and Certs with expiration dates must be listed on CV
  • Anyone with malpractice must provide a NPDB
  • Anyone with legal issues must provide supporting documents
  • Availability
  • 1st possible date they would be available?
  • How many shifts would they be able for per month?
  • What month is their schedule completely open?
  • Any shift preferences (Day, Night, Blocksetc)?
  • Any dates in the next 3-6 months they cant work?
  • Any Holidays in the next 3-6 months they would be open to work.?
  • If provider does not match to all requirements on the Job Order, you must request permission from MSP Coordinator prior to submitting a Name Clear submission.
  • The attached ICA must be used for any provider that is approved for credentialing. This version is also in Trio under your Name & then by clicking Help. Please do not utilize any past versions.
