Job Title: Licensed Practical Nurse (LPN) - Long-Term Care

Job Summary: We are currently seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our long-term care facility. As an LPN in the long-term care setting, you will be responsible for providing high-quality nursing care and support to residents who require long-term assistance with their healthcare needs. Your primary role will be to collaborate with the interdisciplinary team to assess, plan, implement, and evaluate resident care. This position requires strong clinical knowledge, excellent communication skills, and a dedication to enhancing the overall well-being of our residents.

Responsibilities:

  1. Resident Care Planning: Collaborate with the interdisciplinary team to develop individualized care plans based on residents' comprehensive assessments, medical histories, and healthcare goals.
  2. Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accuracy, adherence to medication protocols, and monitoring residents for any adverse reactions or side effects.
  3. Direct Nursing Care: Provide direct nursing care to residents, including wound care, catheterization, administering injections, monitoring vital signs, and assessing residents' overall health status.
  4. Care Coordination: Communicate and collaborate with physicians, registered nurses, therapists, social workers, and other healthcare professionals to ensure comprehensive and coordinated care for residents.
  5. Documentation and Reporting: Maintain accurate and up-to-date medical records, including nursing assessments, care plans, medication administration records, progress notes, and incident reports, ensuring compliance with regulatory requirements.
  6. Supervision and Support: Provide guidance and supervision to Certified Nursing Assistants (CNAs) and other support staff, ensuring the delivery of safe and quality care to residents.
  7. Resident and Family Education: Educate residents and their families about their healthcare conditions, medications, treatments, and self-care techniques, promoting resident empowerment and self-management.
  8. Infection Control: Adhere to infection control policies and procedures, including proper hand hygiene, personal protective equipment usage, and isolation precautions, to prevent the spread of infections among residents and staff.
  9. Quality Improvement: Participate in quality improvement initiatives, such as audits, performance improvement plans, and staff training programs, to enhance the overall quality of care provided in the long-term care facility.
  10. Emotional Support: Provide emotional support to residents and their families, demonstrating empathy, compassion, and active listening skills during challenging times.

Qualifications:

  • Valid and current Licensed Practical Nurse (LPN) license in the state of practice.
  • Minimum of 1 years of experience as an LPN in long-term care or a related healthcare setting.
  • In-depth knowledge of nursing principles, procedures, and practices, particularly in the long-term care context.
  • Proficient in medication administration, wound care, catheterization, and other nursing procedures.
  • Strong assessment and critical thinking skills to develop and implement effective care plans.
  • Excellent communication and interpersonal skills to interact with residents, families, and the interdisciplinary team.
  • Familiarity with electronic health record (EHR) systems and proficiency in computer literacy.
  • Ability to prioritize tasks, multitask, and adapt to changing resident needs in a fast-paced environment.
  • Strong organizational skills and attention to detail to ensure accurate documentation and reporting.
  • Willingness to work flexible schedules, including weekends, evenings, and holidays as required.
