Job Summary:
We are seeking a compassionate and highly skilled Registered Nurse to join our long-term care facility. As a Registered Nurse in the long-term care setting, you will be responsible for providing comprehensive medical care and support to residents who require long-term assistance with their daily living activities. Your primary objective will be to ensure the physical, emotional, and social well-being of our residents while upholding the highest standards of nursing care. This position requires exceptional clinical knowledge, excellent communication skills, and a genuine commitment to providing quality care for our residents.
Responsibilities:
- Assessing Residents: Conduct comprehensive assessments of residents' health status, including physical, mental, and emotional conditions, and develop individualized care plans based on their needs.
- Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accuracy, safety, and adherence to established protocols.
- Monitoring Vital Signs: Regularly monitor and document residents' vital signs, such as blood pressure, heart rate, temperature, and oxygen levels, and promptly report any significant changes to the appropriate medical personnel.
- Care Coordination: Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and other healthcare professionals, to ensure coordinated and holistic care for residents.
- Resident Advocacy: Serve as an advocate for residents, ensuring their rights and preferences are respected, and provide emotional support to residents and their families during challenging times.
- Wound Care: Assess and provide appropriate wound care management, including dressing changes, infection control, and monitoring healing progress.
- Health Education: Educate residents and their families on various health-related topics, including disease prevention, medication management, nutrition, and self-care techniques.
- Documentation and Reporting: Maintain accurate and up-to-date medical records, including nursing assessments, care plans, progress notes, and incident reports, ensuring compliance with regulatory requirements.
- Infection Control: Adhere to infection control policies and procedures, including proper hand hygiene, personal protective equipment usage, and isolation precautions to minimize the risk of infections among residents and staff.
- Quality Improvement: Participate in quality improvement initiatives, such as audits, performance improvement plans, and staff training programs, to enhance the overall quality of care provided in the long-term care facility.
Qualifications:
- Valid and current Registered Nurse (RN) license in the state of practice.
- Minimum of 1 years of experience in long-term care or a related healthcare setting.
- In-depth knowledge of geriatric nursing principles and practices.
- Proficient in conducting comprehensive assessments and developing individualized care plans.
- Strong clinical skills, including medication administration, wound care, and infection control.
- Excellent communication and interpersonal skills to effectively interact with residents, families, and the interdisciplinary team.
- Ability to prioritize and multitask in a fast-paced environment while maintaining attention to detail and accuracy.
- Demonstrated ability to handle challenging situations with empathy, professionalism, and composure.
- Familiarity with electronic health record (EHR) systems and proficiency in computer literacy.
- Willingness to work flexible schedules, including weekends, evenings, and holidays as required.
