WHAT I AM ACCOUNTABLE FOR:
Effective case management of service users with substance use issues
To carry out comprehensive substance use assessments including assessment of personal strengths and identification of any risk factors
Formulate, monitor and review risk assessments and risk management plans that effectively safeguard and protect service users and families/carers from harm
To develop person centred recovery plans using a strengthsbased approach in partnership with service users and others involved in their support network
Maintain a high standard of case notes and prepare and present narrative reports as required
Effective delivery of Group intervention sessions
Consistent, timely and accurate collection, recording and reporting of data and information relating to a client treatment journey
Ensuring achievement of key performance indicators relevant to your role
Effective multiagency working with statutory and nonstatutory agencies in relation to the wider presenting needs and complexities of service users
Deliver and support harm reduction interventions, including needle exchange and access to physical health care including BBV and health screening
Ensuring all paperwork and recording in accordance with our policy and procedure and follow information governance guidelines.
