  • This is a new PACE location to open in summer/fall 2024.
  • Asbury Health Partners provides easily accessible, high-quality primary care services to independent-living residents at Asbury Methodist Village and community members aged 50 and older. The 2,739-square-foot primary care site features the latest technology, state-of-the-art examination rooms and a staff of experts in senior care.
  • The practice is conveniently located on the Asbury Methodist Village campus, allowing providers to coordinate care and recommendations with on-campus services such as wellness, rehabilitation, assisted living, and skilled care. Lab and ECG services are provided on-site. Specimens or blood samples that need to be tested are sent to Holy Cross Hospital.
  • Office hours are Monday through Friday from 8:00 am 4:30 pm. The practice provides 24-hour, on-call coverage to ensure that Asbury residents are able to reach a skilled provider regardless of when a health issue arises.

  • Under the supervision of the Executive Director.
  • Provides general medical direction and supervision of the medical aspects of participant care
  • Responsible for the delivery of participant care, clinical outcomes, and oversight of the Quality Assurance Performance Improvement (QAPI) program.
  • Responsible for the development and maintenance of medical policies and procedures/standards and protocols
  • Provides direction and supervision to related Primary Care Providers (PCPs) and APPs.
  • Provides leadership and guidance to the program colleagues and providers in the local community
  • Need to have a driver's license, proof of insurance, and means of transportation
  • Proficient in Microsoft Suite (Word, Excel, PowerPoint, etc.), in addition to basic knowledge of email, calendaring systems, and other software as required to perform the essential functions of the job
  • Excellent organizational, customer service, flexibility, and problem-solving skills
  • Willingness to travel within the region periodically as needed
  • Conduct history and physical on new participants and event-based and periodic reassessments. Develop medical treatment plan and provide medically necessary services in the range of locations in which participants may reside.
  • Interview and evaluate applicants to PACE Organization and their caregivers and participate in the Intake and Assessment decision.
  • Discuss each participant's medical conditions and prognosis with him/her and their caregiver to assist them in expressing their wishes and planning their future care.
  • Execute assignments in a culture that is shared and collaborative across all divisions within TH PACE.
  • Collaborate in the nursing treatment program of participants.
  • Establish and/or maintain as a member of the leadership team a plan to achieve operating goals.
  • Participate as an active member of PACE Organization interdisciplinary team. Educates and advises the team regarding the implications of diagnoses, cognitive and physical function and treatment options.
  • Participate in team and family conferences and decision-making processes.
  • Educate participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff.
  • Supervise the health care and treatment program as developed by the interdisciplinary team.
  • Perform physical examinations, order and evaluate appropriate laboratory and diagnostic tests, and record results in a systematic manner.
  • Effective skills in physical assessment and chronic disease management for frail older adults.
  • Desire to work in a creative alternate health care setting.
  • Able to manage changing priorities according to participant needs.
  • Participates in provider on-call rotation as required.
  • Minimum of 1 year experience working with the frail or elderly population is required.

  • Provider must be interested in Perm (not accepting Locums)                                                            
    • Board Certified in Internal Medicine or Family Medicine - REQUIRED
    • Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start) - HIGHLY PREFERRED
    • MD license (or willing to obtain) - REQUIRED
    • Clean Malpractice - HIGHLY PREFERRED
    • CDS Certification (by time of start) - REQUIRED
    • DEA (by time of start) - REQUIRED
    • BLS (by time of start) - REQUIRED
    • Previous supervisory experience (add to "Experience" field) - REQUIRED
