Location: South Park, Pennsylvania
Schedule: Friday-Sunday (3 Admissions Daily) + 1 Admin/Flex Day (Mon or Thurs)

Are you a caring and organized Registered Nurse who loves autonomy and direct patient care?
Join an extraordinary home health care team in Brentwood, PA, as a Home Health Admission RN in a vital role where you'll manage new patient admissions, create personalized care plans, and help individuals heal where they feel safest at home.

Required Qualifications:

• Associate Degree or higher in Nursing

• 🩺 Active RN License in Pennsylvania (PA)

• Valid Drivers License and reliable transportation

• At least 1 year of experience with:

  • EMR / EHR systems

  • Practice Partner EMR or equivalent

• One of the following certifications:

  • AHA Basic Life Support (BLS)

  • AHA Advanced Cardiac Life Support (ACLS)

⭐ Preferred:

• Please provide your PA RN license number with your application

• Strong time management and communication skills

• Experience with home health admissions and charting

🩺 Key Responsibilities:

• Conduct three patient admissions per day (Fri-Sun)

• Build and implement individualized care plans

• Collaborate with clinical teams for seamless transitions

• Use mobile tools and EMR to chart and document care

• Manage your time independently and meet documentation deadlines

• Serve as a key liaison between patients, families, and care teams

Schedule Overview:

• Friday-Sunday: 3 patient admissions per day

• 1 Flex/Admin Day/Week (preferably Monday or Thursday)

• No daily shift time restrictions enjoy flexibility
